Nothing on this website is medical advice, a diagnosis or a treatment plan. Reading this site or sending us a message does not create a doctor and patient relationship. A relationship begins only after you have been seen by Dr. Holli and have completed our new patient paperwork. If you think you have a medical emergency, call 911 or go to the nearest emergency room right away.
Care is by appointment only. You can book through our online scheduling page. Booking is handled by a third-party service, which has its own terms and privacy policy. Appointment times, cancellation terms and fees are explained at the time of booking and in your new patient paperwork.
Our contact form is for general questions, such as hours, services and scheduling. Please do not send sensitive health details, insurance numbers or other private medical information through the form or by regular email, because these methods are not fully secure. We will reply to your note, and we may ask you to share health details in person or through our secure patient forms.
We list the insurance plans we work with for your convenience. Coverage and benefits vary by plan, so please check with your insurance company before your visit. Listing a plan on this site does not guarantee coverage or payment.
